Please print, review, and complete the forms below, and bring them to your first visit.
If you plan to utilize your insurance, please bring your medical insurance card and ID. Each session is typically 50 minutes.
If the primary patient is a minor (under age 18), please complete the following forms: Client Registration Form, Informed Consent, Informed Consent for Psychotherapy, and the Child Adolescent Individual Form. The legal guardian or parent will need to sign the forms.
